Hospice at Home Team

Rotherham Hospice

Rotherham Hospice’s Hospice at Home team delivers specialist palliative and end-of-life care to people in the place they call home, 24 hours a day, seven days a week.

In 2025/26, the team completed 12,320 home visits, recorded 38,102 calls and provided more than 1,718 hours of telephone support. Their work helped avoid 1,363 hospital admissions and 1,428 GP appointments, representing a minimum avoided NHS activity value of £1.28m using public NHS and PSSRU unit costs.

The team’s impact is deeply human. They support people to remain at home, help families feel safer and less alone, respond at moments of fear and uncertainty, and reduce avoidable pressure on the wider NHS.

They are skilled, compassionate, practical and courageous, delivering exceptional frontline care in people’s homes at some of the most important moments of their lives.
